Description: Overview - Social Worker (Remote)Salary Range: $70,000.00 - $80,000.00
Are you looking to make a difference by providing high-quality service with a personal touch that impacts the lives of World Trade Center Health Program members? Would you like to be part of a highly-skilled clinical team including home-based Social Workers focused on supporting individuals in maximizing their health and productivity? ResponsibilitiesAs a Social Worker, you will work independently in your home office setting while being part of a supportive nationwide team. You will provide continuity of care for assigned patients in the acute and post-acute settings. Provide/evaluate the biopsychosocial impact on the patient's plan of care. Evaluate the patient's ability to progress throughout the continuum of care. Work collaboratively in communication with physicians, case managers, and other members of the multi-disciplinary care team to effect timely and appropriate patient management and the progression of the care plan-psychosocial assessment and interventions. Understand pre-acute and post-acute resources. Provide coordination of services and act as the key liaison between the patient, the patient's family, and the interdisciplinary healthcare members. The social worker will also be responsible for assisting members requiring benefits counseling. Social Worker Qualifications

  • Bachelor's (BSW) or Master's degree in social work (MSW)
  • Valid and current relevant state license
  • Working knowledge of assessing and addressing the specific needs of the targeted population
  • 3+ years related social work experience with at least 1 year working with chronic health conditions
  • Identification, diagnosis, or treatment of Post-Traumatic Stress Disorder (PTSD), preferred
  • Effective interpersonal, communication, and problem-solving skills
  • Ability to maintain a calm, supportive attitude during crisis or potential crises
  • Attention to detail, timetables, and commitment to completing tasks
  • Experience with Microsoft Windows and computer savvy
  • Must be well organized, efficient, and able to work independently and within a team
  • Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and Internet Router in established home office
  • Must have Excellent Communication Skills via Phone, E-Mail, Text, Verbal, and Documentation Skills and provide 24-hour follow-up to all communication
